Back-to-School Information: Health Forms

Good evening, CCS families:

If your student will need to take any medications or have any medical procedures completed during the school day, authorization from a medical provider is required. Please take the required form to your child’s medical provider to have it completed prior to the first day of school. For more information, please contact your child's school nurse.
