This letter needs to be sent home with the Application for Participation Packet.

This document needs to be completed for those who are atleast 8 years old.  It should only be complete for the ones who were on the list that Emily Riley sent you.  If you have not received a list, contact Emily Riley at cabarrus@sonc.net.
